Support Contact Tracing Efforts
We encourage all community members to engage in contact tracing efforts. If you test positive for COVID-19, please use this form to identify and inform your close contacts: https://www.vdh.virginia.gov/content/uploads/sites/182/2020/04/VDH_Slow_Spread_COVID19.pdf
If you are a close contact of a person who has tested positive for COVID-19, please start to quarantine and stay home for 14 days after your last exposure, monitoring yourself for symptoms.

Regional Roanoke Valley Community Health Assessment Survey: Carilion Clinic, in partnership with Healthy Roanoke Valley (HRV) and the Roanoke City and Alleghany Health Districts, is conducting another Community Health Assessment (CHA) of the Roanoke Valley. Key themes from the Community Health Assessment will be used to develop programs and initiatives to address community needs. We need your help gathering input! The first step of the CHA process is assessing the needs of communities through the Community Health Assessment Survey. Please share the survey widely in order to help us gather this valuable input; the survey takes around fifteen minutes to complete. In addition, if you would like to request paper surveys for your organization to distribute, please complete this short form. Individuals that complete the survey will be entered into a raffle for a $100 grocery gift card.
